Our qualified team offers medical, emotional, and spiritual support. We are available 24/7 by phone and in person to offer guidance, education, and resources.

Covered 100% by Medicare, Medicaid, and most insurance plans. Private insurances such as Kaiser, Providence, Legacy, and OHSU programs are accepted as well.

What does Forest Park Hospice cover?

Hospice Covers:

1. Supplies - Incontinence, grooming, wound care, medical supplies, bed linens, Nutritional supplements like Boost, thickened liquids, and more.
2. Medical equipment delivered to your door within 24 hours or less
3. Medications related to the hospice diagnoses and anything related to pain managment, comfort (including antibiotics) delivered to your door by our pharmacy.
4. Holistic Hospice Team:

• Hospice Medical Director for urgent medication changes and advice 24/7
• Registered Nurse Case Manager to offer in-person medical oversight and symptom management
• Hospice aide to assist in bathing and/or light housekeeping needs
• Social worker for coping skills and resources for end-of-life arrangements and caregiver support
• Music therapy for pain management, stress relief, and therapeutic counseling needs
• Spiritual counselor for spititual guidance, arrangements, and coping skills
• Bereavement Counseling for up to 13 months
• Volunteer support

Full Circle After Care program

We partner with professionals to support families after a loved one passes, helping with tasks like closing social media accounts, canceling Social Security benefits, managing trust-related matters, and more—at no cost to the family.

Arbor Program

To honor each life, we plant a tree in a National Forest for every patient in our care. Families receive a card letting them know where their tree has been planted; creating a lasting, living tribute.

Vigil Support

Forest Park Hospice never wants you to feel alone in this process. That's why we coordinate Vigil Support to ensure our team is there for you when you need it the most.

Nail Care

We offer certified nail care—even for complex conditions—ensuring comfort and attention to every detail.
