About This Provider
SeaStar provides several services beyond just in home care. Care management is a
higher level service, coordinating care and resources for all aspects of a
person's life including legal, financial, environmental, health and more. At
SeaStar, our care managers all have at minimum a Master's degree in social work
in order to maintain the quality of education and experience our clients
deserve. Services we provide include: Assessments and care plans Monitoring
quality of care Accessing resources Providing vetted referrals Placement
assistance Care coordination Crisis intervention and advocacy Wellness checks
24/7 Availability We will also serve as a health care representative for those
who have no family to serve in that capacity or serve as an end of life doula.
