About This Provider

Samaritan Evergreen Hospice serves patients in the familiar surroundings of their own homes, skilled nursing facility, adult foster home and assisted living facilities. Samaritan Evergreen Hospice serves Linn, Benton, Marion, Polk, Lincoln and parts of Tillamook counties. While most hospice care is provided in a patient’s home, there are times when a patient needs to come to an inpatient facility for acute symptom management. The Samaritan Evergreen Hospice House serves this purpose.
Hospice is a philosophy of care for people of all ages in the last stages of a terminal illness. Common terminal diseases may include cancer, end-stage cardiac disease, chronic obstructive pulmonary disease (COPD), end-stage dementia (including Alzheimer’s disease) and neurological disorders.
Hospice does not hasten death. It affirms life and regards dying as a normal process.
Whenever possible, hospice patients stay in the familiar surroundings of their own homes, in an atmosphere of dignity and support for the patient and the family.
Hospice care is covered by Medicare, Medicaid, most commercial and private health insurance plans, and by worker’s compensation. If needed, our staff can assist you with insurance coverage information.
Our dedicated and professional hospice team helps patients and families by offering:

Our staff and volunteers support patients and family members through the final stages of illness. Ongoing contact and support for the family is offered through our bereavement program.

Samaritan Evergreen's Hospice House:
As the only local hospice house serving patients in Benton, Linn, Lincoln, Marion, Polk and Tillamook counties (and one of only three hospice houses in Oregon,) Samaritan Evergreen Hospice House in Albany is an inpatient facility for patients who are near end of life. Samaritan Evergreen Hospice is a local, not-for-profit service that has served the area for nearly 35 years.
Samaritan Evergreen Hospice strongly supports the goal of many helping patients stay wherever they call home up until their final days. Our hospice house offers a place for managing symptoms, providing respite to a caregiver and offering room and board.
At end of life, most people choose a home-like setting such as our hospice house instead of staying in a hospital. As part of Samaritan Health Services, we offer seamless coordination with other medical providers.
