1. Needs Assessment
A detailed conversation to understand where your loved one may need extra support and which areas of daily life are currently manageable.
2. Review of Activities of Daily Living (ADLs)
An evaluation of essential activities—such as bathing, dressing, cooking, mobility, and transferring—to determine where assistance may be required.
3. Identifying Opportunities for Support
Based on the assessment, our Care Manager will identify care options and services that could best support your family’s needs.
4. Home Safety Check
A walk-through of the home to highlight any potential safety hazards, including trip risks, lack of grab bars, improper use of extension cords, or other safety concerns.
5. Social Assessment
A review of possible physical or emotional risks related to isolation or loneliness, ensuring the care plan addresses overall well-being.
6. Care Coordination Plan
Discussion of health concerns, medication routines, and other factors that may need to be incorporated into a coordinated care approach.
7. Care Consult Recommendations
A review of the assessment findings along with professional recommendations. Together, we’ll explore the hours and types of care that would best support your loved one.
