Provider Service

Adult Care Home

Private residences that provide a home-like setting, and serve 5 or less residents. The caregiver may perform several functions, such as personal care, housekeeping, activities and group meals. There are three levels of Adult Foster Care Home licenses.

Assisted Living

Housing that provides an independent lifestyle for older adults but with personal assistance and support available; most have private living space or bedroom and some may offer a kitchenette.

Home Care

Home Care is support by a caregiver or sitter in a clients home or community in order to provide companionship or assistance with personal care or errands.

Home Health

Home Health Care is a wide range of health care services provided in the home for an illness or injury. These services can include Skilled Nursing as well as Physical, Occupational, and Speech Therapy. Home Health Care is usually covered by Medicare and other insurances when ordered by a physician for homebound patients.

Hospice

Hospice care treats the person and symptoms of the disease, rather than treating the disease itself. A team of professionals work together to manage symptoms so that a person’s last days may be spent with dignity and quality, surrounded by their loved ones. Hospice care is also family-centered – it includes the patient and the family in making decisions. Hospice is a covered benefit by Medicare and other insurances.

Life Plan Communities

Life Plan Communities include a a continuum of care from independent living, assisted living and skilled nursing services all on one campus.  An endowment fee in addition to a monthly maintenance fee may be expected. Also called Continuing Care Retirement Communities (CCRCs)

Skilled Nursing Facility

Nursing Homes, skilled nursing and intermediate care for the very frail who are dependent on nursing care; many provide short-term rehabilitation for residents needing rehab between hospital and home.
