A hospital discharge, new diagnosis, recent fall, medication change, or increasing need for support can leave individuals and families wondering: “What happens when no one else is there?”

SafetyLine helps close that gap.

We are a Washington-based, family-owned personal emergency response provider that helps people of all ages stay safer, more independent, and connected to assistance—at home and throughout the community.

SUPPORTING SAFER TRANSITIONS AND REDUCING READMISSION RISKS

The first 30 days after discharge can be especially challenging. Medication mismanagement, missed doses, falls, and delayed access to assistance may contribute to complications and avoidable hospital readmissions.


SafetyLine provides additional support for these common post-discharge risks:

• Medication dispensers can release medications at scheduled times, helping individuals follow their prescribed medication routine and reduce missed, early, or incorrect doses.

• Live reminder calls and device-based reminders can prompt individuals to take medications, eat meals, check blood sugar, attend appointments, or complete other important wellness tasks.

• Personal emergency response devices give individuals a direct way to request assistance following a fall, sudden change in condition, or another urgent situation.

• Fall-detection options may automatically alert the monitoring center when a fall is detected, providing another layer of protection when the individual may be unable to press the button.

• Two-way communication connects the individual with a trained emergency operator who can contact family, caregivers, emergency services, or other designated responders based on the situation.

For discharge planners and social workers, SafetyLine can strengthen the transition from hospital or rehabilitation to home by addressing safety, medication adherence, and access-to-help concerns before discharge.

For community outreach programs and care providers, we offer a trusted resource for individuals who live alone, have chronic health conditions, are recovering after hospitalization or surgery, experience mobility challenges, or live with Alzheimer’s, dementia, autism, developmental disabilities, or other support needs.

For individuals and families, SafetyLine provides reassurance that help is available at the push of a button—24 hours a day.

Our dedicated team works with individuals, families, caregivers, discharge planners, social workers, case managers, healthcare providers, home-health agencies, senior living communities, and community organizations to identify an appropriate solution and coordinate timely service.

Our intake team will help explore available funding options, including Medicaid waiver programs, WA Cares Fund, DDA Assistive Technology, Presumptive Eligibility, PACE, Providence Elder Place, Labor & Industries, long-term care insurance, private insurance, private pay, and veteran discounts.

Our goal is to strengthen continuity of care, encourage adherence to care plans, reduce avoidable risks, and help individuals remain safely connected to assistance wherever daily life takes them.
